Appointment Policy
Patients are responsible for managing and keeping track of their scheduled appointments. Appointments can be conveniently accessed and reviewed through the “My Appointment" feature on the website at www.spinehub.co .

Free cancellation and rescheduling cut off time is 3pm prior to appointment day. 

A "No Show" is defined as missing an appointment without prior notice. No Show, cancellation and rescheduling made after cut off time will incur processing fee. Patient may be entitled to full refund in the event of unforeseen circumstances with justifiable proof.

The company retains the right to cancel appointments if deemed necessary due to exceptional circumstances. 

Late Arrival Policy
Patients are advised to arrived to appointments before their scheduled time. New patients are advised to arrive at least 15 minutes before their scheduled appointment. Established patients are advised to arrive 5 minutes before their scheduled appointment. This allows enough time for the registration process to be completed before the actual appointment.

In the event of late arrival, the patient will be given the option of either being seen that day as a walk-in, if schedule permits, or rescheduled for a later date. This process will ensure patients that do arrive on time are seen in a timely manner.

Informed Consent

I hereby request and consent to appropriate chiropractic case management for me (or for the person named below, for whom I am legally responsible) at Spinehub Chiropractic. The following points of information are explained to me:

1. The purpose of chiropractic care is to optimize health by facilitating neurological and biomechanics integrity of the spine, which allows maximum expression of the body’s innate recuperative abilities.

2. I understand that chiropractic services will be provided by licensed chiropractors of chiropractic and complementary care advises by chiropractic assistants under the supervision of chiropractors who are employed by Spinehub Chiropractic.

3. Spinehub Chiropractic uses only chiropractic methods that are taught in accredited chiropractic colleges, appropriate techniques will be selected for my care based upon standard professional protocols. My diagnosis and all proposed procedures will be explained to me and implemented only with my approval. Alternative and adjunctive recommendations for health promotion will be discussed with me.

4. Chiropractic adjustments are exceedingly safe when applied properly. However, I understand there are some risks to care including, but not limited to discomfort, fractures, and sprains. I do not expect the chiropractor to anticipate and explain every risk and complication, but I will rely on the chiropractor’s best judgement to protect my best interest. No guarantees of cure have been implied or given.

5. A small force is introduced into the spine during a chiropractic adjustment that may lead to temporary musculoskeletal discomfort. This is usually minor and transient.

6. The chiropractor will discuss any further risks inherent for my particular case during a report of findings, and will document this discussion in my case record. Any questions or concerns that I may have will be address at this time. I understand that I am an active participant in my chiropractic care, and that I am encouraged to bring up questions or express any concerns. 

7. I give my permission for Spinehub Chiropractic to communicate by telephone or email regarding matters of chiropractic care, appointment reminders or scheduling.

8. Spinehub Chiropractic is compliant with state, national, and professional body regulations, and take all reasonable precautions to safeguard your privacy in all matters. I understand that any concern I have regarding privacy and safety of my health information may be discussed with my chiropractor.

9. I understand that my clinical data may be anonymously used for research purposes.

10. By signing below, I affirm that I have read this consent document, and I agree to its provision. I intend this document to cover the entire course of care now and in the future.

11. I reserve full right to refuse care or withdraw my consent and discontinue care at any time.

For caregiver/guardian/parent,

I hereby authorize the chiropractor to request, obtain, review and inspect any and all information bearing upon my dependent’s health and relevant to any such decisions to be made respecting such treatment. The chiropractors is to make professional decisions regarding assessment and treatment for said dependent with or without my presence.

Payment and Credit Account Features

Users can top up their credit accounts, which can then be used to confirm appointments. We accept cash, TnG E-wallets, FPX online banking, debit and credit cards for topping up the credit account.

Processing Fee

A processing fee will be incurred for any cancellation or rescheduling made on the scheduled appointment day. Changes made prior to the appointment day will not incur a processing fee.

Appeal Process

Users may appeal the deduction of the processing fee within 7 days from the appointment date. To initiate an appeal, users must send an email to info@spinehub.co and provide a letter including the following information:

• User name
• Identification number
• Contact number
• Related appointment date and time
• Reason for Appeal
• Supportive documents
• Signature

If an appeal is successful, the processing fee will be returned to the user's credit account. The company reserves the right to make the final decision on the outcome of all appeal requests.

Withdraw

Withdrawals from the credit account are feasible and are exclusively processed through bank transfers. To request a withdrawal, users may send an email to info@spinehub.co and provide a letter including the following information:

• Amount of credit to be withdrawn

Credit Transfer

Users can transfer credit to another user's account. Credit transfers will be processed within 5 working days. To initiate a transfer, users may send an email to info@spinehub.co and provide a letter including the following information:
